- Please download and complete the membership form and send it with your check
- click on this link and go to file/download: PSOV Membership Application form
- if you cannot download the form please include the following information with your check:
- Date, Name, Email, Address, and Telephone #
- Regular Membership $40
- Student Membership $20
- Hardship Membership $20
- Patron Membership $60
